Medical Check-up for Centre for Foundation Studies (CFS) IIUM Gambang Campus

1. Please note well that the medical check up is COMPULSORY for all new students and it will be done at IIUM Sejahtera Clinic (ISC) Gambang Campus. Medical check up done at any other place than ISC will not be accepted.

2. Medical check-up form can be downloaded from https://www.iium.edu.my/centre...

3. Fill in the form carefully and bring it along on the day of your appointment to ISC.

4. Do not forget affixing your latest passport size photograph in the box provided on the form.

5. Chest x-ray is not compulsory unless instructed by the examining medical doctor during the medical check-up at ISC. It can be done at our centre (at an additional cost).
