RENEWAL OF IIUM GROUP HOSPITALIZATION AND SURGICAL INSURANCE (GHSI) SCHEME 2020/2021 FOR IIUM STAFF

Commencement                                  :        1st June 2020

Insurer                                                 :        Syarikat Takaful Malaysia Berhad (STMB)

Third Party Administrator (TPA)    :        PM Care Sdn Bhd.

Kindly be informed, that the University will renew the Group Hospitalization & Surgical Insurance (GHSI) scheme with Syarikat Takaful Keluarga Malaysia Berhad (STMB) for all IIUM Staff.   This benefit is given by the University in addition to the present University medical benefit scheme, (which allows staff and dependants to be treated at General/Government and University Hospitals)

 

With the GHSI, staff can seek medical services and treatment at private hospitals up to the maximum amount as specified in Plan 1, 2, 3 or 4.  The University is subsidizing only RM250.00 for all Plan undertaken.  The staff may decide for additional upgrading of the GHSI and/or with the inclusion of family members (dependants), which is self borne through employee salary deduction.

Accordingly, the deduction will be base on the choice of Plan decided by the staff on monthly basis.

Kindly submit the duly completed form (enclosed), by INDICATING CLEARLY, the Plan OPTED, before or on the 30th MAY 2020.

  • Salary Deduction Schedule according to Plans
  • GHSI Form (to be duly filled and return)

Should the Office of Health and Wellness Centre DO NOT receive the GHSI form, BEFORE or ON the date, it is automatically deemed that the staff has opted for Plan 4 (Employee Only).
